Award of a military decoration not previously awarded can be requested according to instructions in Army Regulation 600-8-22, Military Awards, paragraph 1-14, found at: http://www.apd.army.mil/pdffiles/r600_8_22.pdf If the desired result is not achieved through this process and this remedy is exhausted, application can be made to the Army Board for Correction of Military Records. Please include a copy of the Army Military Awards Board’s decision.

  5. Correction of Retirement Points. Information on retirement points correction can be found at https://www.hrc.army.mil/tagd/retirement points accounting system rpas. This site requires an Army Knowledge Online (AKO) username and password or Common Access Card (CAC) to access this website.
  6. Change of name, gender, or SSN on discharge or military records. If you have a name, gender, or social security number (SSN) change after discharge from the military, even if it is court ordered, the name and gender on your military records will not be changed since they are historical documents which record facts during the time you served in the military. If you need a certificate to show your current name or gender and that you served in the military under another, you can request such from the National Personnel Records Center, Military Personnel Records, 1 Archives Drive, St. Louis, Missouri 63138.   7. Home of Record correction. According to AR 600-8-104, table 5-2, item 25 and AR 601-280, table 11-1, item 3 – Home of Record means the place (city and state or country) recorded as your home when you first enlisted, appointed or commissioned. This can only be changed if there is a break in service of more than one day. To correct an error, enlisted personnel must apply to apply to Commander, US Army Human Resource Command, Service Center, 1600 Spearhead Division Avenue, Fort Knox HK 40121; or email Askhrc.army@us.army.mil ; or call 1-888-276-9472. Your S1/unit eMILPO clerk, Personnel Services NCO (PSNCO), and your Personnel Services Battalion (PSB) can find information on correcting and updating this data in the THE ELECTRONIC MILITARY PERSONNEL OFFICE FIELD USER’S GUIDE which is available through Army Knowledge Online at https://www.us.army.mil/suite/doc/32374255. The Army Board for Correction of Military Records (ABCMR) cannot consider your request until you have exhausted this administrative remedy. The ABCMR procedures can take up to 12 months so would not be an immediate solution.
  8. Removing Unfavorable Information (GOMOR, Letter of Reprimand) from records. Before you can apply to the Army Board for Correction of Military Records for consideration for removal of unfavorable information from your file, such as a letter of reprimand, you must exhaust administrative remedy (appeal process) defined in AR 600-37, Unfavorable Information, Chapter 7. If after exhausting administrative remedy, you still feel that there is an error or injustice in your record, you may apply to the ABCMR. You may complete an online application at https://actsonline.army.mil and send the signature page and evidence as instructed by the online program, or you may print a blank DD Form 149 from the Army Review Boards Agency website at https://arba.army.pentagon.mil/index.htm and mail it to the address shown on the reverse of the form. As part of your evidence please provide a copy of documents showing that you have exhausted all administrative remedy.
  9. Removal of an Article 15 from records. Before you can apply to the Army Board for Correction of Military Records for consideration for removal of an Article 15, you must exhaust administrative remedy (appeal process) defined in Army Regulation 27-10, Military Justice, paragraph 3-37 & 4-41, Table 3-2 (Removal of Records of Non-judicial Punishment from Military Personnel Files); Army Regulation 600-8-104, Military Personnel Information Management/Records, Table 2-1 (under numbered form 2627 (DA), and Army Regulation 600-37, Unfavorable Information, Chapter 7. If after exhausting administrative remedy, you still feel that there is an error or injustice in your record, you may apply to the ABCMR. You may complete an online application at https://actsonline.army.mil and send the signature page and evidence as instructed by the online program, or you may print a blank DD Form 149 from the Army Review Boards Agency website at https://arba.army.pentagon.mil/index.htm and mail it to the address shown on the reverse of the form.   10. Correction of Military Records. Title 10, United States Code, Section 1552, is the law governing correction of military records.
